Learning Objectives:
Apply the screening guidelines for diabetes and chronic kidney disease in populations at risk to identify these
diseases at an earlier stage, in order to prevent or slow complications.
Integrate evidence based guidelines in complicated patients with multiple chronic diseases to develop clinical action plans
designed to prevent or minimize adverse long term outcomes.
Use evidence based intervention to slow the deterioration in glomerular filtration rate and delay the onset of ESRD.
Institute team care for patients with diabetes to control risk factors, screen for complications, improve follow up care, appropriately refer to specialists, and provide access to supportive clinical resources.
